I was fortunate enough to be named a DCU Community Hero recently, and this was a thank-you package that was part of that. There’s no fully defined, concrete path toward becoming a Community Hero, but there is more information on what the team is generally looking for here:

Every once in a while, you may have seen topics highlighting a random box of goodies people received in the mail that included a letter thanking them for being a “Community Hero”.
